Abstract

Purposes: The authors studied the clinical significance of SPECT and MRI in relation to TMJ pam This study was designed (1) to investigate the frequencies of manifestation of organic changes of painful TM joints on SPECT and MRI, (2) to study the correlation between tbe visual estimation result of the amount of Tc-99m MDP uptake on SPECT and the grade of joint effusion on MRI, (3) to measure the relative amount of Tc-99m MDP uptake by painful and painless TM joints on SPECT, and (4) to investigate the prevalence of painful TMJ according to the grade of joint effusion Materials and Methods Sixty-three adult patients(126 TMJs), who presented with TMJ pain were selected for the study SPECT was obtained on each patients and semiquantitative evaluation was made by forming a formula from the counts of the TMJ reiative to the activity of the skull MRI was taken with 50 of 63 patients and the amount of the J010t effusion was assessed and graded Results SPECT detected the pathophysiologic change of TMJ in 38 of 52 joints (73%) with spontaneous pam MRI was a little less sensitive and 34 joints (65%) had high signal intensity on MRI The joints with spontaneous pam showed higher uptake than painless joints
Conclusion It can be concluded that SPECT bone scintigraphy is the promising, noninvasive imaging test of choice to evaluate subjective pain symptom of patient with temporomandibular disorders
